Description
Vynca is hiring a Lead Care Manager to serve as a client’s primary point of contact and coordinate care across medical, mental health, substance-use, social, housing, and community-service needs. The role develops care plans, advocates with providers, connects clients to resources, documents progress in EMR and CRM systems, completes reporting, and supports member access to care. It is a hybrid position requiring travel throughout the Los Angeles area up to five days per week, with candidates required to reside within a 20-mile radius of the assigned territory. The role requires at least two years of experience supporting vulnerable populations, a valid driver’s license, reliable transportation, and strong communication and computer skills; bilingual English/Spanish ability is preferred.
