Vynca is hiring a Lead Care Manager in Orange County, California, to serve as a client’s primary point of contact and coordinate care across medical, mental health, substance-use, social, housing, and community-service needs. The role develops care plans, advocates with providers, connects clients to resources, documents progress in EMR and CRM systems, completes reporting, and may arrange transportation. It is a hybrid position requiring travel throughout the Orange County area up to five days per week, with candidates expected to reside within a 20-mile radius of the assigned territory. The role requires at least two years of experience supporting vulnerable populations, a valid driver’s license, reliable transportation, and strong communication and computer skills; bilingual English/Spanish ability is preferred.
