Westhab is hiring a Case Manager in Queens, New York to assess clients’ needs, develop individual service plans, coordinate housing and supportive services, provide advocacy and crisis intervention, document case activities, and meet placement quotas. The role requires at least one year of case management experience with homeless, mentally ill, substance-abuse, or low-income populations, strong relationship-building and organizational skills, PC proficiency, and the ability to work flexible evening and weekend schedules; a bachelor’s degree in social work or human services is preferred, while proof of education is required.
