Description
Catholic Charities is hiring a Case Manager to support at-risk and gang-impacted youth and their families through outreach, needs assessment, resource linkage, case management, school and transition services, program activities, advocacy, and coordination with schools, community partners, the San Jose Police Department, and Juvenile Probation. The role also includes administrative reporting, electronic documentation, after-hours support, and flexible scheduling, with a BA preferred and extensive equivalent experience or knowledge accepted.
