Description
The Children’s Mental Health Case Manager assesses children and families, develops functional assessments and individual treatment plans, coordinates community services and referrals, monitors service delivery, documents client records, and communicates with providers and supervisors. The role may be assigned to one or more counties under Minnesota Prairie County Alliance and may be performed in clients’ homes, community locations, or the agency office. A bachelor’s degree in a behavioral science or related field is required; four years of human services experience and two years as a children’s mental health case manager are preferred. The position offers medical, dental, and vision insurance, retirement plans, paid time off, and a salary of $23.50-$26.00.
