Description
The Case Manager Clinician provides therapeutic services, case management, and care coordination for individuals served through Hope Network. The role includes psychosocial assessments, individual and group therapy, person-centered planning, treatment-plan development, service coordination, admissions and discharge support, crisis intervention, and clinical documentation. Candidates need a master's degree in a relevant field, Michigan licensure or eligibility for licensure, and the ability to participate in an on-call rotation.
