Wellspring in Louisville, KY is seeking a full-time Case Manager for their Home for Good initiative. This role involves providing case management, connecting individuals experiencing chronic homelessness to permanent supportive housing, and offering ongoing support to ensure they can retain housing. Additional Information

Preferred Qualifications: Experience working directly with adults in a social service, mental health service, substance abuse service, and/or homeless service setting.
Knowledge of the theories of human behavior and the principles and techniques associated with the provision of recovery-based case management.
Strong ability to establish and maintain an effective working relationship with adults with a serious mental illness coming from homelessness, colleagues and agency/community providers/Metro/Home for Good committees.
Ability to communicate effectively and respectfully, both orally and in writing.
Consistently demonstrate a strong respect for the dignity of all individuals.
Working knowledge of community resources and ability to facilitate coordination activities.
