Description
The Case Manager provides in-home, strengths-based family assessments and services to families enrolled in the Family Preservation Program. The role develops and monitors individualized service and treatment plans, co-facilitates case conferences, connects families to community resources, maintains client records, and collaborates with multidisciplinary teams and supervisors. It requires a bachelor's degree in a relevant field, one year of case management experience or education in lieu of experience, English and Spanish communication skills, and travel within Los Angeles County.
