Description
The Case Manager will work with formerly homeless men at the Cromwell Avenue Safe Haven in Bronx, New York, assessing client needs, developing individual service plans, connecting clients to services and benefits, providing supportive counseling, documenting activities, and monitoring progress. The role may supervise direct-care staff and requires an associate degree or 60 college credits in Human Services, a high school diploma or GED with two years of relevant shelter experience, and verbal, written, and basic computer communication skills.
