ADAPT Community Network is hiring a full-time Case Manager for a residential setting in Bronx, New York. The role is responsible for developing, implementing, and monitoring Life Plans and Individualized Service Plans for people with developmental disabilities, coordinating services across residential, clinical, and day program partners, supporting families and guardians, and maintaining regulatory compliance and documentation. The position requires a bachelor's degree in Human Services or a related field, at least one year of experience working with individuals with developmental disabilities, strong communication and organizational skills, and proficiency with Microsoft Office and electronic record systems. Compensation is $28.34 to $29.43 per hour after probation, with industry-leading benefits for full-time employees.
