Description
Southwest Network is hiring a Case Manager for its adult outpatient clinics. The role assesses and develops individualized service plans, coordinates behavioral, medical, and other providers, supports recovery and self-sufficiency, and helps members achieve employment, community activity, independent living, and social network goals. The position offers $20-$25 per hour depending on experience, medical, dental, and vision coverage, paid time off, retirement matching, and other benefits. Applicants need a behavioral health or related degree, or an alternative education path with related experience, current CPR, First Aid, and Therapeutic Options certifications, a valid Arizona driver's license, and strong communication and organizational skills.
