Description
Axiom Care is hiring a full-time Specialty Case Manager to support clients and residents in substance use treatment, behavioral health, and recovery housing programs. The role manages client caseloads, develops individualized treatment plans, documents care in electronic medical records, coordinates community resources, supports crisis de-escalation, and assists with program activities. The position requires computer and electronic medical record proficiency, active CPR and First Aid certification, an Arizona Fingerprint Clearance Card for some assignments, and at least one year of case management experience with specified populations.
