Humana is hiring a Case Manager for its Inclusa/Humana team to serve members in the Wisconsin Family Care program in the Hayward/Park Falls area. The role assesses health and safety needs, develops care plans, coordinates with a Field Care Nurse, conducts social assessments and quarterly in-person visits, arranges non-medical support services, evaluates risks, and maintains case documentation. It requires a bachelor's degree in human services or a related field plus relevant experience, a valid driver's license, and personal vehicle liability insurance. The position is primarily remote with up to 40% travel, Monday through Friday from 8:00 a.m. to 4:30 p.m. CST, and pays $53,700 to $72,600 annually.
