Description

CAMBA is hiring a full-time Project Coordinator to provide intensive case management and client advocacy for adults with serious mental illness enrolled in a Health Home. The role includes assessments, care planning, referrals, crisis intervention, field visits, appointment escorting, benefit applications, progress documentation, and community outreach. The position is based at 2244 Church Ave, Brooklyn, with a hybrid schedule available, and pays $62,593 annually with health and dental insurance and other benefits.

Additional Information

Minimum Education: A master’s degree in a qualifying human-services field with one year of experience, or a bachelor’s degree in a qualifying field with two years of experience, or a CASAC credential with two years of experience, or a bachelor’s degree or higher in any field with three years of experience or two years of experience as a Health Home care manager serving the SMI or SED population.
Required Previous Experiences: One year of experience with a master’s degree in a qualifying field
Two years of experience with a bachelor’s degree in a qualifying field
Two years of experience with a CASAC credential
Three years of experience with a bachelor’s degree or higher in any field
Two years of experience as a Health Home care manager serving the SMI or SED population
Preferred Qualifications: Bilingual English and Spanish
