Description
The Case Manager I supports families, children, and individuals experiencing or formerly experiencing homelessness by developing care plans, coordinating housing and health services, providing outreach and crisis intervention, facilitating referrals, assisting with benefits, and maintaining client records. The role works under direct supervision, collaborates with community partners and health providers, and may require travel to clients’ homes, shelters, or other outreach situations. It requires a high school diploma or GED, Basic Life Support certification within 30 days, a Colorado driver’s license, and 1–2 years of relevant experience is preferred; the schedule is Monday through Friday on an eight-hour day shift with flexibility for occasional schedule changes.
