Description
The Medical Case Manager provides case management services to HIV-positive individuals under the supervision of a Ryan White Program Manager. The role assesses client eligibility, develops and monitors individualized care plans, coordinates medical and community services, advocates for clients, conducts home visits and appointments, documents services, and maintains confidentiality and safety standards. Required qualifications include a bachelor’s degree in social work or a related human-services field with one year of paid post-degree direct-service experience, or an RN degree with one year of paid experience; current BLS certification, a Louisiana driver’s license, and the ability to pass an auto insurance carrier review are also required.
