Description
The Case Manager I supports families, children, and individuals experiencing or formerly experiencing homelessness by developing care plans, coordinating housing and health services, providing outreach and crisis intervention, facilitating referrals, assisting with benefits, and maintaining client records. The role works under direct supervision, requires a high school diploma or GED and Basic Life Support certification, and is scheduled Monday through Friday on an eight-hour day shift from the Denver-metro area office.
