CNS Healthcare is hiring a Case Manager to provide community-based case management and recovery support for individuals with substance use disorders and co-occurring conditions. The role includes assessment, weekly interventions, transportation assistance, recovery groups, documentation, treatment planning, medication coordination, advocacy, and collaboration with health and community providers. Candidates need a QMHP or SUD professional qualification, relevant substance-use and treatment knowledge, strong communication and collaboration skills, and intermediate computer and Microsoft Office experience; state licensure is preferred. Compensation ranges from $44,000 for unlicensed candidates to $75,315 annually for fully licensed candidates, with medical, dental, vision, retirement, paid time off, holidays, parental leave, and student loan forgiveness benefits.
