Acenda Integrated Health is hiring a Case Manager for its Integrated Case Management Services program in Gloucester. The role provides face-to-face case management, assessment, care planning, community resource linkage, discharge planning, post-discharge follow-up, advocacy, and documentation for clients, including those with serious and persistent mental illness. The position requires on-site hospital and community work, flexible scheduling, and a valid driver's license.
