Sutter Health is hiring a full-time Care Management RN to coordinate care and plan transitions for acute-care patients, including emergency-department patients. The role collaborates with physicians, nurses, social workers, payers, quality teams, and community resources to promote timely progression to appropriate care levels, prevent unnecessary admissions or readmissions, and support patient self-determination. The position requires a California RN license upon hire, recent relevant experience, and offers day/evening shifts with every-other-weekend requirements.
