Catholic Charities is hiring a full-time Case Manager III – Blue Plus Team for its Aging and Disability Services in Elliot Park with a hybrid work arrangement. The role provides initial and annual assessments, service planning, client advocacy, benefit and resource coordination, case documentation, education, crisis support, and mentorship to older adults and individuals with disabilities. It requires a relevant bachelor’s or master’s degree or a bachelor’s degree plus at least one year of social services experience, a valid driver’s license, CPR, and the ability to work with diverse, low-income, and vulnerable populations.
