Description
Vynca is hiring a hybrid Case Manager, internally titled Lead Care Manager (LCM), for its Enhanced Care Management team. The role serves as the client’s primary care coordinator, develops care plans, connects members with health and social services, advocates with providers, documents progress in EMR and CRM systems, and completes reporting. It requires at least two years of experience supporting vulnerable populations, a valid driver’s license, reliable transportation, and availability Monday through Friday from 8:30 a.m. to 5:00 p.m. Pacific Time, with flexibility for evenings and weekends. The position is based in the Orange County area, requires travel up to five days per week, and is limited to candidates residing within a 20-mile radius of the assigned territory.
