Description
Breaking Ground is hiring a Case Manager, Connect to Care to provide outreach and case management services to adults experiencing homelessness, including psychosocial assessments, benefits assistance, referrals to medical and substance-use treatment, housing applications, service planning, crisis intervention, and recordkeeping. The role reports to the Social Services Supervisor, has a 35-hour work week, and requires a bachelor’s degree or equivalent work experience, two years of experience working with homeless adults, and strong computer skills.
