Description
The Case Manager Trainee provides client-centered, strength-based, trauma-informed rehabilitation and recovery services to adults and transition-age youth with mental illness and, in some cases, co-occurring substance abuse disorders. The role includes developing and maintaining treatment plans, providing supportive counseling, delivering psychiatric rehabilitation and daily-living skills training, monitoring medication, coordinating medical and community services, conducting crisis assessments, maintaining case records, and supporting discharge planning. The position may include on-call responsibilities and, as assigned, floor-supervision or coverage of other counselor or case manager duties. Candidates need a bachelor’s degree in a human services field, or an associate’s degree with two years of relevant experience, or a high school diploma with four years of human services experience; qualified candidates lacking the experience may be hired as trainees at a reduced salary until requirements are met.
