Description
The Part-Time Case Manager will assess applicants, conduct functional assessments and interviews, coordinate admissions and placement, support residents in maintaining independence, maintain care records, coordinate with providers and families, and assist with alternative-care planning. The role is part-time, three days per week on the day shift with Wednesdays required, and pays $25.00 per hour. A master’s degree in social work is preferred, while a bachelor’s degree in social work or a related field and at least two years of experience working with older adults are required.
