Description
The Case Manager Pathway of Hope provides crisis intervention and case management services to clients at The Salvation Army's Concord Shelter and Pathway of Hope program. The role assesses clients, develops individualized service plans, coordinates community resources, monitors client progress, trains and supervises student interns, maintains confidential records, and supports shelter operations and reporting. The position requires a bachelor's degree in social work, behavioral science, or a related field, or equivalent training and experience, and is based in Concord, North Carolina.
