Description
The Case Manager coordinates and facilitates care for patients at Kindred Hospital Northland, working with interdisciplinary teams, physicians, payors, and external resources. The role provides care planning, implementation, evaluation, advocacy, referrals, utilization review, and discharge planning while supporting continuity of care and compliance. It requires an LPN/LVN or RN, a social work degree with licensure, healthcare professional licensure, fluent English, computer skills, and one year of healthcare experience.
