The Centers is hiring a full-time, non-exempt Case Manager for its Permanent Supportive Housing (PSH) program across multiple Cleveland-area locations. The role provides strength-based case management to individuals experiencing chronic homelessness and behavioral health challenges, with responsibilities spanning client engagement, service planning, coordination with multidisciplinary teams, documentation in an electronic medical record, and support for community-based and onsite program activities.
