Description
The Case Manager will provide direct, housing-first, trauma-informed services to young adults ages 18–24 in the Rapid Re-Housing Program, managing a case load of approximately 15–20 clients. Responsibilities include housing navigation, documentation in HMIS and Salesforce, life-skills and financial-management training, housing-plan support, community-resource referrals, problem-solving, and transportation to appointments. The role requires at least one year of relevant experience, a bachelor’s degree, a valid driver’s license, and strong communication and collaboration skills.
