Description
The Case Manager provides direct patient care, discharge planning, care coordination, utilization review, appeals and grievance support, and transitions of care for Independence Health System patients. The role requires a registered nurse license in Pennsylvania, three years of clinical or recent case management experience, strong communication and documentation skills, knowledge of payor benefits and utilization management, and the ability to work rotating shifts, standby, on-call, and travel locally between Independence Health locations.
