Description
The Case Manager provides case management services to seniors and adults with disabilities, including intake, home-visit assessments, care-plan development and implementation, service coordination, monitoring, reassessment, and termination when appropriate. The role also maintains client records, reports incidents to Adult Protective Services, conducts outreach and community education, participates in multidisciplinary case conferences and training, and contributes to program evaluation and quality assurance. A bachelor's degree in a related field or equivalent case management experience is required, while elder abuse training or experience is strongly preferred; bilingual ability and the ability to travel are also required or preferred.
