Description
The Case Manager II provides casework services to CDCR clients in the STOP Area Six Program, including intake, initial placement, case planning, progress monitoring, community-provider coordination, transportation, documentation, and aftercare planning. The role may require travel to community-based providers, including overnight stays, and involves maintaining client and provider records, communicating with parole agents and CDCR personnel, and supporting new-hire training. Qualifications include a high school diploma or equivalent with Substance Use Disorder Counselor registration/certification, or a bachelor’s degree in social work, psychology, or a related field with First Aid and CPR certification; a minimum of two years of case management experience with the criminal justice population is required.
