Description
The Care Manager provides PRN support for the Integrated Care Management program at Upper Valley Medical Center. The role coordinates care across the healthcare continuum, reviews medical records, supports discharge and transition planning, addresses utilization and care-delay issues, prepares CMS and third-party payer documentation, makes referrals, participates in risk management and quality measures, and mentors healthcare providers. The position requires a BSN, an active Ohio State Registered Nurse license, and three to five years of recent acute care experience combined with knowledge of utilization and case management principles.
