Description
SMOC is hiring a Case Manager to provide initial and ongoing assessment, individualized service planning, referrals, home visits, group facilitation, crisis intervention, and case documentation for episodically homeless individuals with relapse histories. The role requires a bachelor’s degree or five years of experience with the target population, substance abuse experience, a valid driver’s license, reliable transportation, and insurance eligibility. It reports to the Program Manager, requires travel within the state, and offers a hybrid work option of 0–20% remote work.
