Hope Solutions is hiring a temporary, full-time Case Manager for its Continuum of Care Rental Assistance Program to provide home-based case management to formerly homeless adults. The role assesses consumer needs, develops service plans, coordinates medical, mental health, recovery, financial, legal, and housing services, supports eviction prevention, and maintains documentation. It requires a BA in social work or a related field, at least two years of human-service experience, an operational vehicle, valid California driver’s license, and the ability to work across Contra Costa County. Compensation is $28 to $32 hourly, with Kaiser health and dental coverage, a 403(b) match, paid holidays, vacation, and life insurance.
