Description
The Case Manager, Hartford Transitional House (HTH) provides supervised residential case management for men in a criminal justice reentry program. The role conducts resident intake, LSI-R assessments, service planning, referrals, progress and discharge reporting, PREA assessments, documentation, data entry, and coordination with community services. It requires a bachelor’s degree plus one year of assessment and case management experience or equivalent experience, strong communication and computer skills, a valid driver’s license, reliable transportation, and a schedule including two late days per week.
