Description
The Case Manager, Waterbury Transitional House (WTH) provides supervised residential case management for men in a reentry program. The role conducts intake, LSI-R assessments, service planning, referrals, progress and discharge reporting, behavior monitoring, PREA assessments, documentation, data entry, and coordination with community services. It requires a bachelor’s degree plus one year of assessment and case management experience or equivalent experience, strong communication and computer skills, a valid driver’s license, reliable transportation, and availability for two late days per week.
