Breaking Ground is hiring a Case Manager, Transitional Housing to provide case management services to approximately 15 homeless adults, including those with mental illness and/or substance use disorders. The role supports clients in transitioning to permanent housing through assessments, benefits assistance, service planning, referrals, documentation, crisis intervention, and appointment support. The position reports to the Clinical Coordinator and has a 35-hour work week.
