Description
Fraser is hiring Case Managers to support individuals on CADI/BI and DD waivers. The role provides care coordination, monitors health and safety needs, resolves service gaps, and advocates for clients by connecting them to community resources. DD Case Managers must have a relevant bachelor’s degree, one year of experience, a developmental-disabilities course, a valid Minnesota driver’s license, and pass a DHS background study; CADI/BI Case Managers must have a relevant bachelor’s degree or a bachelor’s degree plus one year of social-service experience. The position is fully remote after the initial onboarding period, requires travel to meet clients, and is based in Minnesota’s Dakota, Hennepin, and Ramsey County areas. Starting pay is $50,000 to $52,000 per year plus a $1,500 hiring bonus for external candidates.
