Description
SMOC is hiring a Case Manager to provide case management and rehousing services to veterans, including intake assessments, housing action plans, advocacy, crisis intervention, transportation coordination, safety inspections, and coordination with community and Department of Veterans Affairs providers. The role serves approximately 15 veterans per caseload, works Monday through Friday from 8:00 AM to 4:00 PM for 35 hours per week, and requires a bachelor's degree or at least three years of experience in human services or a related field, along with a valid driver's license and the ability to meet insurance standards.
