The Doe Fund is hiring a Case Manager for its newest permanent supportive housing program serving homeless individuals living with HIV/AIDS. The role provides supportive case management, assessments, service planning, healthcare referrals, counseling, crisis intervention, benefit assistance, community-service coordination, health and independent-living activities, rent and budgeting support, unit inspections, reporting, and resident education. The position requires a bachelor’s degree in social work or a relevant field, at least three years of post-bachelor experience, HIV/AIDS and related population knowledge, strong case management and communication skills, and availability to cover evening hours.
