Description
The Case Worker will implement, monitor, and evaluate case management activities for a human services program serving individuals dealing with homelessness, health care referrals, and other social service needs. The role conducts intake interviews and assessments, counsels clients, helps secure housing and benefits, maintains case files, coordinates with community and public agencies, and supports families and clients in stabilizing housing, healthcare, finances, and other needs. An associate’s degree or equivalent education and four years of applicable experience are required; a baccalaureate degree, relevant experience, and bilingual English-Spanish or English-Mandarin/Cantonese ability are preferred. The position offers medical, dental, and vision coverage.
