Description
The Case Worker will implement, monitor, and evaluate case management activities for a human services program under the supervision of a Program Director. The role assesses clients and families dealing with homelessness, health care, benefits, and other social service needs; conducts intake interviews, counseling, housing and employment assistance, medical referrals, case planning, documentation, and follow-up; and coordinates with community partners and public agencies. An associate’s degree or high school diploma/GED with four years of applicable experience is required, a baccalaureate degree is preferred, two years of relevant experience is preferred, and bilingual English-Spanish ability is preferred. The position includes medical, dental, and vision coverage.
