Description
Oscar is hiring a remote CCM Case Manager for its Complex Case Management team. The role coordinates care across inpatient, outpatient, emergency, and home-care settings; partners with members and providers on personalized care plans; assesses social determinants of health; connects members with community resources; provides crisis intervention and advocacy; and educates members about health conditions, hospital discharges, procedures, and prescriptions. Candidates need at least two years of clinical care-management experience, two years of experience with chronic medical conditions, one year of care-coordination and navigation experience, and either an active unrestricted United States RN license, an active compact multistate unrestricted RN license, or an MSW from a nationally accredited school. The position is open to candidates residing in the listed states and offers base pay of $67,813 to $89,005 per year in most locations, with medical, dental, and vision benefits.
