Columbus House is hiring two full-time CCR Case Managers for its Permanent Supportive Housing Department. The role provides case management to adults experiencing homelessness, helping clients find and maintain housing, connect with medical, mental health, substance abuse, benefits, and employment resources, coordinate with agencies and landlords, document cases, and create person-centered service plans. The position is scheduled Monday through Thursday from 8:00 a.m. to 4:00 p.m., pays $25.50 to $27.50, and offers medical, dental, vision, retirement, paid time off, and paid holidays.
