Description
Swan Hospice is hiring a Certified Home Health Aide (CHHA) with hospice experience to provide compassionate personal care and support to patients in skilled nursing facilities, assisted living facilities, and private homes. The role assists with mobility and range-of-motion exercises, toileting, transfers, positioning, activities of daily living, medication reminders, light housekeeping, meals, vital signs, and reporting and documenting patient-condition changes under Hospice Nurse supervision.
