1bios is hiring a full-time, 1099 Care Coach to work remotely from the United States and support patients with chronic conditions. The role involves managing patient panels, conducting health assessments, following care plans, reviewing remote monitoring alerts, creating electronic escalations, coordinating with care teams, and supporting patient health goals. Candidates must hold Certified Medical Assistant, LPN, or LVN credentials, be comfortable with high-volume phone and computer work, and have a private workspace; RPM or CCM experience is preferred but not required.
