1bios is hiring a full-time, 1099 Care Coach to work remotely from the United States and support patients with chronic conditions through phone and electronic communication. The role manages patient panels, performs health assessments, follows care plans, creates alert escalations, coordinates with care teams, and supports health goals. Candidates must be bilingual in English and Korean, hold Certified Medical Assistant, LPN, or LVN credentials, and be comfortable with high-volume phone outreach. Experience with Remote Patient Monitoring or Chronic Care Management is preferred but not required.
