Description
The Certified Social Services role at Mission Pines is a hybrid position supporting residents in a skilled nursing facility through psychosocial care, discharge planning, resource coordination, documentation, and resident programs, while also providing direct CNA care such as activities of daily living, vital signs, transfers, and safety support. The role requires long-term care experience, social services certification, a high school diploma or equivalent, and current CNA certification; experience or training in social services, counseling, psychology, or a related field is preferred.
