Description
Heart to Heart Hospice is seeking a Chaplain to independently manage a caseload of hospice patients and families, conduct spiritual assessments, provide pastoral counseling and crisis intervention, support bereavement families, participate in interdisciplinary care planning, and collaborate on spiritual, pastoral, and ethical issues. The role requires advanced pastoral knowledge through theological education, clinical pastoral education, board certification, or ordained ministry; at least one year of healthcare or death-and-dying experience; interfaith and multi-belief competency; computer and EMR proficiency; strong English communication; and the ability to manage an independent caseload.
